What is Xarelto used for?

Xarelto tablets 20mg are used to treat and prevent the formation of blood clots (thrombosis) in conditions where an embolism may result and could be life threatening.  Blood clot formation is a normal response to blood vessel damage to stop bleeding.  However, a blood clot (thrombus) can also form in situations where blood flow is disturbed or slows down due to reduced mobility, heart failure or atrial fibrillation (irregular heart beat).  If a clot becomes detached from the vessel it can travel through the circulation and this is a thromboembolism.  Blood clot formation in the veins is called a venous thrombosis or deep vein thrombosis, because they usually occur in the deep veins of the leg or pelvis and can become lodged in the lungs causing a pulmonary embolism.  If a clot form in an artery or in the heart, it can block the blood supply to the heart tissue and cause a myocardial infarction (heart attack); or it can travel to the brain and cause a stroke.  Xarelto tablets 20mg are used to prevent venous thromboembolism (VTE) following hip or knee replacement surgery when there is increased risk of blots clots due to immobility; also to treat and prevent recurrent deep vein thrombosis (DVT) and pulmonary embolism (PE).  Xarelto tablets 20mg are used to prevent stroke and systemic embolism (blood clots in peripheral blood vessels) due to non-valvular atrial fibrillation and other risk factors, such as congestive heart failure, hypertension (high blood pressure), increasing age over 75 years, diabetes mellitus, prior stroke or transient ischaemic attack (mini stroke).  

How does Xarelto work?

Xarelto tablets 20mg contain rivaroxaban, which is a highly selective direct inhibitor of Factor Xa, a clotting factor in the coagulation cascade.  Factor Xa is a serine protease, an enzyme that converts prothrombin to its active form thrombin.  The coagulation cascade involves the interaction of a series of coagulation factors, which are mostly enzymes, resulting in the generation of plug formed by a fibrous protein (fibrin) that combines with a clump of aggregated platelets and becomes a blood clot or thrombus.  This cascade is activated to prevent bleeding under normal conditions, following injury or trauma to a blood vessel.  It can also be activated if blood becomes stagnant and is no longer free flowing such as when muscles are paralysed during surgical anaesthesia or if the heart pumping mechanism fails such as due to atrial fibrillation.  The intrinsic pathway is initiated when collagen is exposed in damaged blood vessel walls and the extrinsic pathway is activated by tissue factor released from blood vessels as a result of trauma to the endothelial cell lining of the blood vessel.  The activation of Factor Xa is the common link in the coagulation cascade.  Factor Xa plays a pivotal role in the coagulation pathway by directly converting prothrombin to thrombin, which leads to the generation of fibrin resulting in clot formation.  The activation of factor Xa causes amplification of coagulation with a burst of thrombin generation.  Therefore, inhibition of Factor Xa is an effective target for preventing thrombosis, by rapidly inhibiting the burst of fibrin generation and thereby preventing formation of a clot from both the intrinsic and extrinsic pathway.  Several of the coagulation factors, are dependent on Vitamin K as a cofactor in their production and include factors VII, IX, X and II; these are the target of other anticoagulants like warfarin, which can take time to become effective and may need dose adjustment.  Rivaroxaban in Xarelto tablets 20mg is fast acting with high bioavailability when taken orally, is not affected by dietary vitamin K and is effective at a fixed dose.

What is Glucobay used for?

Strone-100 injection is used to prevent miscarriage and help an early pregnancy to continue where there have been previous repeated unexplained miscarriages; or where the body has not produced enough progesterone to maintain the pregnancy due to a luteal phase defect. Strone-100 injection is also used to help with certain infertility treatments, including In-Vitro Ferilisation (IVF) or Gamete Intra-Fallopian Transfer (GIFT). In selected cases Strone-100 injection helps increase the chances that the fertilised egg will successfully implant into the endometrium. Another use for Strone-100 injection is to treat dysfunctional uterine bleeding, which is abnormal bleeding and includes heavy bleeding (menorrhagia), irregular bleeding, spotting mid-cycle, post-menopausal bleeding or no bleeding (amenorrhea). How should Strone-100 be used?

You will be given Strone-100 by intra-muscular injection into the buttock, by your nurse or doctor. The dose you are given and how frequently will depend on what you are being treated for and your doctor’s recommendation. For a threatened miscarriage you will be given one injection immediately, followed by daily injections for 7-10 days. To prevent a miscarriage or to assist with infertility treatment you will be given an injection usually twice weekly from around day 9 up to 12-14 weeks of pregnancy. For uterine bleeding disorders, you will be given an injection daily for 5-10 days until around two days before your period is due.

What is Pimafucort used for?

Pimafucort Ointment is used for the short-term treatment of superficial skin conditions (dermatoses), that have become or are at risk of becoming infected with a bacterial or fungal infection.  These skin conditions include dermatitis (inflammation of the skin), seborrhoeic dermatitis (caused by over active sebaceous glands), eczema also called atopic dermatitis as there is a predisposition due to an inherited atopic tendency, lichen simplex a form of dermatitis made worse by constant itching and scratching.  Also, psoriasis, autoimmune skin condition characterised by a chronic scaly rash.  Pimafucort Ointment helps relieve symptoms like itching, redness, dryness, crusting, scaling and discomfort common to many skin conditions, particularly where there is inflammation and where there is also the added complication of a bacterial and/or fungal (usually candida) infection.

How does Pimafucort work?

Pimafucort Ointment contains a combination of the corticosteroid hydrocortisone, the antibiotic neomycin, and the antifungal natamycin, that work together to treat inflammatory and infected skin conditions. Hydrocortisone in Pimafucort Ointment is a potent anti-inflammatory with anti-pruritic properties (anti-itching) and works by blocking the action of phospholipase A2, the first enzyme involved in the synthesis of prostaglandins and leukotrienes, which are hormone-like substances involved in the inflammatory process; also cyclooxygenase (COX 1 and COX 2), the enzymes specifically involved in prostaglandin production.  Blocking the production of inflammatory chemicals prevents the swelling and redness characteristic of inflammation and the itching and rash formation that can happen as a result of inflammation.  Hydrocortisone in Pimafucort Ointment also causes vasoconstriction (blood vessels contract), which constricts blood vessels that have become dilated (widened) and leaky, allowing other inflammatory cells into the skin to escalate the inflammatory response.  Neomycin in Pimafucort Ointment is a broad-spectrum antibiotic that is commonly used in creams and ointments and is effective against a wide range of bacteria that cause skin infections. Pimafucort Ointment contains a combination of three active ingredients: the corticosteroid hydrocortisone (10mg/g), the antibiotic neomycin (3.5mg/g), and the antifungal natamycin (10mg/g), that work together to treat inflammatory and infected skin conditions.  It also contains polyethylene oleogel (liquid paraffin with 5% w/w polyethylene).

Treating infected skin conditions with Pimafucort

Pimafucort Ointment contains a combination of the corticosteroid hydrocortisone, the antibiotic neomycin, and the antifungal natamycin, that are used to treat superficial skin conditions, that have become or are at risk of becoming infected with a bacterial or fungal infection. These skin conditions include dermatitis, eczema, seborrhoeic dermatitis, lichen simplex (a form of dermatitis) and psoriasis, an autoimmune skin condition characterised by a chronic scaly rash.  Hydrocortisone in Pimafucort Ointment is a potent anti-inflammatory with anti-pruritic properties (anti-itching) and works by blocking the production of inflammatory mediators like prostaglandins that cause swelling and redness characteristic of inflammation and the itching and rash formation that can happen as a result of inflammation.  Hydrocortisone in Pimafucort Ointment also causes leaky blood vessels to constrict, preventing leakage of inflammatory cells at the site of inflammation.  Neomycin in Pimafucort Ointment is a broad-spectrum antibiotic effective against a wide range of bacteria that cause skin infections and works by blocking the synthesis of essential proteins needed for the bacteria to survive and replicate, which kills the bacteria and prevents spread of infection.  Natamycin in Pimafucort Ointment is an antifungal that works by binding to ergosterol, an important component of the fungal cell membrane which weakens the fungal cell membrane, causing the cell contents to leak out, which kills the fungus.  Pimafucort Ointment helps relieve symptoms of inflammation and prevents spread of bacterial and fungal infections of the skin.

What are the side effects of Pimafucort?

The most commonly reported side effects when taking Pimafucort Ointment include temporary aggravation of the skin lesion, worsening of the skin condition known as rebound effect, skin irritation due to contact allergy to neomycin, thinning or discolouration of the skin, delayed wound healing, discolouration or depigmentation of the skin, reddening of the skin, skin rash, increased hair growth.

When should Pimafucort not be used?

You should not use Pimafucort Ointment if you:

  • are allergic to hydrocortisone, neomycin, natamycin, or any ingredients in Pimafucort
  • have had side effects to other corticosteroids like dermatitis around the mouth or stretch marks
  • are pregnant or are breastfeeding, without discussion with your doctor
  • have a primary bacterial, fungal, viral or parasitic skin infection
  • have ulcers on the skin, wounds or burns

How should Pimafucort be used?

You should apply a small amount of your Pimafucort Ointment to the affected area of skin 2-4 times daily and gently rub into the skin.  Wash your hands before and after use and avoid contact with the eyes. Do not use Pimafucort Ointment on the eye-lids as this can increase risk of glaucoma or cataracts. You should continue to use your Pimafucort Ointment for as long as recommended by your doctor but should not exceed 14 days.   

Pimafucort Ointment should be used for treatment of chronic skin disorders with scaly, dry or fissured skin lesions, and for seborrhoeic dermatitis.

Pimafucort Cream should be used for treatment of acute and subacute skin disorders, particulary if the skin condition is in a hairy area of skin or in skin folds. 

How long should you use Pimafucort?

You should continue to use your Pimafucort Ointment for as long as recommended by your doctor but should not exceed 14 days. 

Missed dose of Pimafucort

If you miss a dose of Pimafucort Ointment apply it as soon as you remember, unless it is time to apply the next dose, then skip the missed dose.  Do not apply a double dose.

How should Pimafucort be stored?

You should store your Pimafucort Ointment below 25°C in a cool dry place.

What is Strone-200 used for?

Strone-200 injection is used to prevent miscarriage and help an early pregnancy to continue where there have been previous repeated unexplained miscarriages; or where the body has not produced enough progesterone to maintain the pregnancy due to a luteal phase defect.  Strone-200 injection is also used to help with certain infertility treatments, including In-Vitro Ferilisation (IVF) or Gamete Intra-Fallopian Transfer (GIFT).  In selected cases Strone-200 injection helps increase the chances that the fertilised egg will successfully implant into the endometrium.  Another use for Strone-200 injection is to treat dysfunctional uterine bleeding, which is abnormal bleeding and includes heavy bleeding (menorrhagia), irregular bleeding, spotting mid-cycle, post-menopausal bleeding or no bleeding (amenorrhea).  Uterine bleeding disorders are usually due to a hormonal imbalance between oestrogen and progesterone and Strone-200 injection is used to help restore this balance.

How does Strone-200 work?

Strone-200 injection contains progesterone, a hormone that is involved in regulating the normal menstrual cycle and is naturally produced by the ovaries during the second phase or luteal phase of the cycle.  It acts on the endometrium (lining of the uterus) converting the proliferating phase to the secretory phase to prepare the endometrium for a pregnancy.  This causes the endometrium, which has thickened in response to oestrogen during the first half of the cycle, to stop growing and start secreting hormones and other proteins needed to maintain a pregnancy, if conception occurs; if not, levels of oestrogen and progesterone fall and the endometrium is shed (menstruation).  Progesterone is produced by the corpus luteum in the ovary for the first few weeks of pregnancy (8-12 weeks) and once the fertilised egg has successfully implanted into the endometrium, progesterone is produced by the developing placenta to continue the pregnancy.  Strone-200 injection helps maintain progesterone at levels that are sufficient for implantation to occur and for a pregnancy to continue to reduce the risk of a miscarriage.  Strone-200 injection also helps maintain sufficient levels of progesterone for increasing the success of assisted fertility treatment for infertility; also progesterone in Strone-200 injection helps restore the balance between oestrogen and progesterone to correct uterine bleeding disorders.

What does Strone-200 contain?

Strone-200 injection contains the active ingredient progesterone a naturally produced hormone involved in the normal menstrual cycle and in maintaining a pregnancy.  Each ampoule contains 200mg progesterone per 2mL in a medium chain triglyceride oil base with benzyl alcohol as preservative.  ie it is Progesterone 100mg/ml but is a 2ml vial containing 200mg 

Treating threatened miscarriage with Strone-200

When should Strone-200 not be used?

You should not use Strone-200 if you:

  • are allergic to progesterone or other progestins or any ingredients in Strone-200
  • have ever had an incomplete or missed abortion/miscarriage
  • have a past or family history of carcinoma of the breast, endometrial carcinoma
  • a history of thrombosis (blood clots) or stroke
  • abnormal genital bleeding of unknown cause
  • have liver disease
  • are taking medicines that interact with Strone-200, including bromocriptine (for menstrual problems), the immunosuppressant cyclosporine.

How should Strone-200 be used?

You will be given Strone-200 by intra-muscular injection into the buttock, by your nurse or doctor.  The dose you are given and how frequently will depend on what you are being treated for and your doctor’s recommendation.  For a threatened miscarriage you will be given one injection immediately, followed by daily injections for 7-10 days.  To prevent a miscarriage or to assist with infertility treatment you will be given an injection usually twice weekly from around day 9 up to 12-14 weeks of pregnancy.  For uterine bleeding disorders, you will be given an injection daily for 5-10 days until around two days before your period is due.

How long should you take Strone-200?

The dose you are given and how frequently will depend on what you are being treated for and your doctor’s recommendation.
