What is Nicotine Replacement Therapy?

Nicotine Replacement Therapy or NRT is the safe administration of nicotine other than by smoking cigarettes and without all the other toxic and harmful chemicals found in cigarette smoke.  Using NRT nicotine can be absorbed into the body in controlled amounts to gradually reduce the body’s dependency on nicotine for the instant reward provided by cigarette smoking that leads to addiction.  This process of slowly weaning off nicotine helps reduce the craving for nicotine while at the same time helping to reduce withdrawal symptoms.  NRT is used to treat and reduce smoking dependence and to provide support and motivation for smoking cessation. 
How does NRT work?

Nicotine in cigarettes is absorbed by the lungs and rapidly enters the blood to provide an instant response that satisfies the craving for nicotine.  NRT will not satisfy the craving as the nicotine takes longer to get into the blood and is intended to provide less nicotine than a cigarette but will help reduce the craving over time and reduce withdrawal symptoms.

Types of NRT

NRT is available in the following forms:

  • Nicotine gum releases nicotine into the mouth where it is absorbed through the mucous membrane in the mouth, by parking the gum in the cheek after a few chews have released the nicotine.  Gum can be used to replace cigarettes up to 12-15 times daily.  There are also different strengths of gum so that heavy smokers can gradually be weaned onto to lower strength gum and eventually by reducing the number of pieces used per day, off the gum altogether.
  • A nicotine inhaler can be used to substitute for cigarettes whenever there is an urge to smoke, using a cartridge with a mouthpiece.  The amount of nicotine released is less than a cigarette but can be used for periods up to 20 minutes.  After about three months the number of cartridges used should be reduced until they are no longer needed.
  • Nicotine containing patches provide continual slow release of nicotine throughout the day and the nicotine is absorbed through the skin on various parts of the body where the patch can be applied, such as the arm, chest or thigh.  Patches last for up to 16 hours and can be worn throughout the day.
