What is Capecitabine used for?

Capecitabine Winthrop tablets 500mg are used as oral chemotherapy to treat various cancers.  These include locally advanced or metastatic breast cancer if other treatments have failed; Dukes’ stage C colon cancer, which is an advanced cancer that has been categorised following surgery as having metastasised to local lymph nodes; first line treatment for metastatic colorectal cancer (cancer of the bowel and rectum); first line treatment for oesophago-gastric cancer (cancer of the stomach and oesophagus).  Capecitabine Winthrop tablets 500mg can be used as monotherapy or in combination with other cytotoxic cancer medications.

How does Capecitabine work?

Capecitabine Winthrop tablets 500mg contain capecitabine, an oral chemotherapeutic drug belonging to the fluoropyrimidine group of drugs that is used to treat a variety of cancers.  Capecitabine itself is not toxic but is a precursor of 5-fluorouracil (5-FU), a cytotoxic metabolite that is activated by specific enzymes that exist in normal cells but in higher levels in cancer cells, thereby producing more 5-FU at the tumour site than in normal cells.  Capecitabine in Capecitabine Winthrop tablets 500mg is an antimetabolite, which means that once metabolised, 5-FU is incorporated into newly synthesised nuclear material and causes metabolic errors that interfere with normal processing of DNA and RNA.  Also 5-FU binds to an important enzyme in the formation of an essential component of DNA, which inhibits the synthesis of DNA.  Both mechanisms contribute to the cytotoxic action of Capecitabine Winthrop tablets 500mg that causes inhibition of cell growth and ultimately cell death of the cancer cells.  Since capecitabine in Capecitabine Winthrop tablets 500mg is a tumour-activated and tumour-selective cytotoxic drug it targets tumour cells and has reduced effect on normal cells. 

What does Capecitabine contain?

Capecitabine Winthrop tablets contain the active ingredient capecitabine (500mg), a cytotoxic drugs used to treat a variety of cancers.  They also contain croscarmellose sodium, microcrystalline cellulose, hypromellose, silica colloidal anhydrous, magnesium stearate, titanium dioxide, talc, macrogol 400, yellow and red iron oxide.  

Treating cancers with Capecitabine

What are the side effects of Capecitabine?

The most commonly reported side effects when taking Capecitabine Winthrop tablets 500mg include, gastrointestinal disorders (diarrhoea, nausea, vomiting, indigestion, abdominal pain), dehydration, stomatitis (inflammation of the mouth and lips, including cold sores and mouth ulcers), fatigue, asthenia (weakness or weariness), headache, dizziness, loss of appetite, anorexia, increased eye watering or irritation, conjunctivitis, skin rash, dry itchy skin, hair loss, hand-foot syndrome or palmar-plantar erythrodysesthesia (tingling, numbness, redness, swelling and pain on the palm of the hands and soles of the feet), fever.

When should Capecitabine not be used?

You should not use Capecitabine Winthrop tablets 500mg if you:

  • are allergic to capecitabine or fluorouracil or any ingredients in Capecitabine Winthrop
  • are pregnant or are breastfeeding
  • have kidney or liver disease
  • have a heart problem or a history of cardiovascular disease, arrhythmias, angina pectoris
  • have known dihydropyrimidine dehydrogenase (DPD) deficiency
  • are taking medicines that interact with Capecitabine, including anticoagulants like warfarin, the anticonvulsant phenytoin, leucovorin for folic acid deficient anaemias, the antiviral sorivudine or analogues like brivudine, allopurinol for gout, antacids like aluminium hydroxide for indigestion, the antipsychotic clozapine 

What medications interact with Capecitabine?

Several medications interact with Capecitabine Winthrop tablets 500mg and should either not be taken while you are taking Capecitabine or only after discussion and instruction from your doctor:

  • Drugs that cause a serious reaction with Capecitabine: anticoagulants like warfarin can cause altered coagulation or bleeding; the antiviral sorivudine or analogues like brivudine can cause fluoropyrimidine toxicity, which is potentially fatal; the antipsychotic clozapine increases risk of agranulocytosis (affecting white cell production in the bone marrow) 
  • Drugs that affect Capecitabine: leucovorin for folic acid deficient anaemias, allopurinol for gout, antacids like aluminium hydroxide for indigestion
  • Drugs that are affected by Capecitabine: the anticonvulsant phenytoin

How should Capecitabine be taken?

You should take your Capecitabine Winthrop tablets 500mg swallowed whole and with food (within 30 minutes of eating) twice daily, morning and evening.  The dose you take and for how long depends on what you are being treated for and your doctor’s recommendation.  In most cases you will take your Capecitabine Winthrop tablets 500mg for the recommended number of weeks followed by a rest period.  You may also be taking other cytotoxic medication as advised by your doctor.  You should continue to take your Capecitabine Winthrop tablets 50mg for as long as recommended by your doctor, which may be for several treatment cycles, depending on how well you respond. 

How long should you take Capecitabine?

You should continue to take your Capecitabine Winthrop tablets 500mg for as long as recommended by your doctor, which may be for several treatment cycles, depending on how well you respond.  

Missed dose of Capecitabine

If you miss a dose of Capecitabine Winthrop tablets 500mg take it as soon as you remember, unless it is time to take the next dose, then skip the missed dose.  Do not take a double dose.

How should Capecitabine be stored?

You should store your Capecitabine Winthrop tablets 500mg below 25°C in a cool dry place.
