What is Nicorette Invisipatch Step 3 used for?

Nicorette Invisipatch Step 3 10mg is the third step of a transdermal delivery system used as a nicotine replacement therapy to help smokers over 18 years quit smoking and withdraw from nicotine dependence1.  Nicorette Invisipatch Step 3 10mg relieves the craving for nicotine and reduces the withdrawal symptoms that smokers can experience when stopping smoking.  These withdrawal symptoms may include anxiety, cravings, increased appetite or weight gain, irritability, dysphoria (state of unease), low or depressed mood, difficulty concentrating, restlessness, insomnia, frustration or anger and decreased heart rate.  Nicorette Invisipatch Step 3 10mg helps provide the motivation to quit smoking and break the nicotine dependence habit but should be used in conjunction with a behavioural support programme for optimum success rate in quitting smoking.  Nicorette Invisipatch Step 3 10mg can be used by pregnant and breastfeeding women trying to quit smoking and can be used alone or in combination with Nicorette gum or inhaler.  Smoking cessation reduces the risk of health problems, caused by the many harmful chemicals in tobacco smoke, including heart disease, stroke and cancer. 

How does Nicorette Invisipatch Step 3 work?

Nicorette Invisipatch Step 3 10mg contains nicotine, an alkaloid found in the nightshade plant family which includes tobacco.  Nicotine is highly addictive and works by binding to specific receptors throughout the nervous system to exert a wide variety of stimulant and depressant effects throughout the body.  Nicotine acts directly on the brain and increases the release of the neurotransmitter dopamine in the brain.  Neurotransmitters are chemicals that allow nerve cells to communicate and dopamine is a neurotransmitter that regulates mood.  When stimulated by nicotine, dopamine creates a feeling of relaxation and euphoria in the reward circuits of the brain, which eventually leads to addiction.  Nicotine also causes a release of adrenaline, a stimulant that increases heart rate, as well as other chemicals which have a variety of physiological effects that contribute to nicotine dependence.  Nicorette Invisipatch Step 3 releases a specified amount (10mg) of nicotine over a period of 16 hours to control the amount of nicotine that enters the blood and helps to wean smokers off nicotine dependence without the unpleasant withdrawal symptoms.  This will help focus on breaking the psychological reliance on the smoking habit, so that the smoker can become used to living without the “feel good” reward provided by smoking and therefore no longer need it.  A clinical study has shown that starting treatment with a higher dose nicotine patch (25mg) for a 12 week treatment period was more effective than a lower starting dose (15mg) for long term cessation of smoking, defined as complete abstinence for one year, particularly for heavy smokers (20 or more cigarettes a day), although increasing the treatment period did not improve the long term success rate2

Nicorette Invisipatch Step 3 contains the active ingredient nicotine (10mg), an alkaloid found in tobacco that is used in the treatment of nicotine dependence, and the relief of withdrawal symptoms associated with smoking cessation.  It also contains triglycerides, medium-chain basic butylated methacrylate copolymer, polyethylenterephthalate film (PET), acrylate matrix (acrylic adhesive solution, potassium hydroxide, croscarmellose sodium, aluminium acetylacetonate), release liner (polyethylenterephthalate (pet) film single side aluminised, both sides siliconized)

What are the side effects of Nicorette Invisipatch Step 3?

The most commonly reported side effects when using Nicorette Invisipatch Step 3 include, local skin reaction at the site of application like itching and redness, dizziness, headache, gastrointestinal upset like nausea, vomiting and discomfort.  Some effects, such as dizziness, headache and sleeplessness may be withdrawal symptoms due to low nicotine levels.

When should Nicorette Invisipatch Step 3 not be used?

You should not use Nicorette Invisipatch Step 3 10mg if you:

  • are allergic to nicotine or any ingredients in Nicorette Invisipatch
  • are pregnant or are breastfeeding, without discussion with your doctor
  • have liver or kidney disease
  • have active duodenal or gastric ulcers
  • have hyperthyroidism (a disorder of the thyroid gland) or phaeochromocytoma (a tumour of the adrenal gland)
  • have heart problems, like angina or palpitations
  • have a had a stroke or heart attack or have high blood pressure
  • have diabetes, without discussion with your doctor as nicotine can affect blood sugar levels
  • have chronic generalised dermatological disorders such as psoriasis, chronic dermatitis or urticaria
  • are a non-smoker, or only smoke occasionally

What medications interact with Nicorette Invisipatch Step 3?

Several medications interact with Nicorette Invisipatch Step 3 and should either not be taken while you are taking Nicorette Invisipatch Step 3 or only after discussion and instruction from your doctor:

  • Drugs that cause a serious reaction with Nicorette Invisipatch: theophylline for asthma, the antidepressant imipramine, the pain medication pentazocine, the dopamine agonist ropinirole, the antipsychotic olanzapine, tacrine for Alzheimer's disease, the antipsychotic clozapine, the antiarrhythmic flecainide.

How should Nicorette Invisipatch Step 3 be used?

You should use your Nicorette Invisipatch Step 3 10mg by applying the patch once daily in the morning and removing it 16 hour later, usually at bedtime.  You should remove the patch overnight to give the body a break from nicotine while sleeping, which is also less likely to cause skin irritation and sleep disturbance.  Dispose of the used patch when you remove it.  You should apply your Nicorette Invisipatch Step 3 to a clean, dry intact area of hairless skin, for example on the hip, thigh, upper arm, or chest and use a different site each day.  You can use the same site again after 2-3 days if there is no skin reaction.  You should use continue to use Nicorette Invisipatch for the full 12 weeks of a recommended smoking cessation treatment programme, starting with the 25 mg Step 2 patch and reducing the strength as follows:

  • Nicorette Invisipatch 25mg Step 1 for the first 8 weeks
  • Nicorette Invisipatch 15mg Step 2 for the next 2 weeks
  • Nicorette Invisipatch 10mg Step 2 for the next 2 weeks

If you smoke 15 or fewer cigarettes a day then you should start with Nicorette Invisipatch 15mg step 2.  At the end of the 12 week treatment programme, you should stop using the patches altogether.

What is the recommended treatment programme for using Nicorette Invisipatch for heavy smokers?

If you smoke 20 or more cigarettes a day, the recommended treatment programme for Nicorette Invisipatch is for 12 weeks and you will be advised to use the patches in 3 steps as follows.

Step 1 - Nicorette Invisipatch 25mg for 8 weeks

Step 2 - Nicorette Invisipatch 15mg for 2 weeks

Step 3 - Nicorette Invisipatch 10mg for 2 weeks.

You should use Nicorette Invisipatch 25mg as Step 1 of a 3-step, 12 week recommended treatment programme.  Only after you have completed 8 weeks using Nicorette Invisipatch 15 mg Step 2 should you then start using Nicorette Invisipatch 15mg Step 2 for a further 2 weeks then complete the programme using Nicorette Invisipatch 10mg Step 3.  After this 3 month period you should be able to quit smoking altogether, if not you may be recommended to try a further treatment programme with Nicorette Invisipatch or to use Nicorette gum or inhaler at the same time when you feel a craving and you can use this combination for 6-12 weeks.  

What is the recommended treatment programme for using Nicorette Invisipatch for medium/light smokers?

If you smoke less than 15 cigarettes a day, the recommended treatment programme for Nicorette Invisipatch is for 12 weeks and you will be advised to use the patches in 2 steps as follows.

Step 2 - Nicorette Invisipatch 15mg for 8 weeks

Step 3 - Nicorette Invisipatch 10mg for 4 weeks.

You should use Nicorette Invisipatch 15mg Step 2 as the first step of the 12 week recommended treatment programme.  Only after you have completed 8 weeks using Nicorette Invisipatch 15mg Step 2 should you then start using Nicorette Invisipatch 10mg Step 3 for a further 4 weeks to complete the programme.  After this 3 month period you should be able to quit smoking altogether, if not you may be recommended to try a further treatment programme with Nicorette Invisipatch or to use Nicorette gum or inhaler at the same time when you feel a craving and you can use this combination for 6-12 weeks.  

How long should you use Nicorette Invisipatch Step 3?

You should continue to use Nicorette Invisipatch 10 mg Step 3 for the full 2 or 4 weeks of the12 week recommended smoking cessation treatment programme, depending on whether you are following the programme for heavy or medium/light smokers.

Missed dose of Nicorette Invisipatch Step 3

If you miss a dose of Nicorette Invisipatch Step 3 10mg apply a patch as soon as you remember, unless it is time for the next patch, then skip the missed dose.  Do not apply a double dose.

How should Nicorette Invisipatch Step 3 be stored?

You should store your Nicorette Invisipatch Step 3 10mg below 25°C in a cool dry place.
