Zapril tablets 0.5mg contain cilazapril, an Angiotensin Converting Enzyme (ACE) inhibitor, used to treat hypertension and congestive heart failure, by widening blood vessels and lowering blood pressure.

Zapril general information

What is Zapril used for?

Zapril tablets 0.5mg are used to treat all grades of essential hypertension, which is high blood pressure when at rest, by lowering blood pressure to normal resting levels; also for renovascular hypertension, which is a form of secondary hypertension caused by narrowing of the arteries in the kidneys.  Although there are usually no symptoms of high blood pressure, lowering high blood pressure reduces risk of stroke, heart attack and end organ damage such as kidney disease.  Zapril tablets 0.5mg are also used to treat congestive heart failure, by helping weakened heart muscles pump more efficiently and reducing feelings of breathlessness on exertion.  They are suitable for treatment of symptomatic heart failure and asymptomatic patients with left ventricular dysfunction, to reduce risk of disease progression and need for hospitalisation in patients with congestive heart failure.  Zapril tablets 0.5mg can be used alone as monotherapy or with a diuretic, which helps get rid of excess water that is retained in body tissues and can cause swelling (oedema).  For treatment of congestive heart failure Zapril tablets 0.5mg are used in addition to treatment with digitalis and/or a diuretic. 

How does Zapril work?

Zapril tablets 0.5mg contain cilazapril a specific and long-acting Angiotensin Converting Enzyme (ACE) inhibitor that prevents the formation of angiotensin II, a hormone that has several actions involved in regulating blood pressure, which is the force needed to pump blood around the body.  Angiotensin II causes salt and water retention by the kidneys, and causes blood vessels to constrict (narrow); this increases blood volume while at the same time reducing blood vessel volume, which increases blood pressure.  Cilazapril in Zapril tablets 0.5mg inhibits the conversion of angiotensin II from angiotensin I, which allows blood vessels to relax and widen, reducing the force needed by the heart to pump blood around the body, reducing its workload and lowering blood pressure.  This also helps weakened heart muscle, such as in congestive heart failure, to work more efficiently and reduces symptoms of breathlessness on exertion.  Cilazapril in Zapril tablets 0.5mg is rapidly absorbed into the blood and rapidly converted to the active form, cilazaprilat; it is effective within two hours and is effective for 24 hours, so that you need only take one tablet a day.

What does Zapril contain?

Zapril tablets 0.5mg contain cilazapril, an Angiotensin Converting Enzyme (ACE) inhibitor that is used to treat hypertension and congestive heart failure.  They also contain corn starch, lactose, sodium bicarbonate, collodial silicon dioxide, sodium stearyl fumarate, Opadry II White Y-30-18037.

Treating hypertension (high blood pressure) with Zapril

What are the side effects of Zapril?

The most commonly reported side effects when taking Zapril include dizziness, headache, cough, hypotension, rash, nausea, diarrhoea, fatigue, dyspepsia and other gastrointestinal disturbances.

When should Zapril not be used?

You should not use Zapril if you:

  • are allergic to cilazapril or any other ACE inhibitor or any ingredients in Zapril
  • are pregnant or are breastfeeding
  • have kidney or liver disease
  • have diabetes, without careful monitoring of blood glucose levels
  • have hereditary or idiopathic angioedema (rapid swelling under the skin)
  • are taking potassium tablets or potassium-containing salt substitutes
  • are taking medications that interact with Apo-Cilazapril, including, potassium-sparing diuretics like spironolactone, lithium for mood disorders, antidiabetics like insulin, non-steroidal anti-inflammatory medicines (NSAIDs) like diclofenac or ibuprofen, lithium for mood disorders, other antihypertensives like methyldopa, calcium entry blockers or beta blockers

What medications interact with Zapril?

Several medications interact with Zapril tablets 0.5mg and should either not be taken while you are taking Zapril or only after discussion and instruction from your doctor:

  • Drugs that cause a serious reaction with Zapril: potassium-sparing diuretics like spironolactone may cause low blood calcium, non-steroidal anti-inflammatory medicines (NSAIDs) like diclofenac or ibuprofen can cause kidney problems
  • Drugs that affect Zapril: other antihypertensives like methyldopa, calcium entry blockers or beta blockers increase the antihypertensive effect
  • Drugs that are affected by Zapril: lithium for mood disorders, antidiabetics like insulin

How should Zapril be taken?

You should take your Zapril tablets 0.5mg swallowed whole with a glass of water, with or without food, once daily, and at the same time each day.  Your dose may be increased depending on what you are being treated for, how well your respond and your doctor’s recommendations.  Zapril tablets 0.5mg helps improve your condition but does not cure it therefore, you should continue to take your Zapril tablets 0.5mg for as long as your doctor recommends.  Zapril tablets 0.5mg may take a few weeks to lower blood pressure and you should keep taking Zapril tablets 0.5mg even if you feel well, as often there are no symptoms for high blood pressure. 

How long should you take Zapril?

Zapril tablets 0.5mg helps improve your condition but does not cure it therefore, you should continue to take your Zapril tablets 0.5mg for as long as your doctor recommends.  Zapril tablets 0.5mg may take a few weeks to lower blood pressure and you should keep taking Zapril tablets 0.5mg even if you feel well, as often there are no symptoms for high blood pressure. 

Missed dose of Zapril

If you miss a dose of Zapril tablets 0.5mg take it as soon as you remember, unless it is time to take the next dose, then skip the missed dose.  Do not take a double dose.

How should Zapril be stored?

You should store your Zapril tablets 0.5mg below 25°C in a cool dry place.
