Zimybe tablets 10/20mg contain ezetimibe and simvastatin, two cholesterol-lowering medications that work by different mechanisms and are used in combination to treat high cholesterol (hypercholesterolaemia) to reduce risk of heart disease. 

Zimybe general information

What is Zimybe used for?

Zimybe tablets 10/20mg are used to lower high levels of cholesterol (hypercholesterolaemia), low-density lipoprotein cholesterol (LDL-C or “bad cholesterol”) and other fats (triglycerides) in the blood; also to increase high-density lipoprotein cholesterol (HDL-C or “good cholesterol”) in adults and adolescents (10 to 17 years).  Zimybe tablets 10/20mg are used in addition to a low-fat diet, when lifestyle changes (diet and exercise) or when use of a statin alone are not sufficient to lower cholesterol in patients with primary hypercholesterolaemia, which may be due to lifestyle (high-fat diet and lack of exercise) or can be inherited (familial hypercholesterolaemia).  This form of hypercholesterolaemia is known as heterozygous and is not always lifestyle dependent.  Zimybe tablets 10mg can also be used for Homozygous Familial Hypercholesterolemia (HoFH), which is an inherited form and is more serious if untreated.  There are usually no symptoms associated with high cholesterol and Zimybe tablets 10/20mg help reduce health risks associated with coronary heart disease (CHD), including heart attack and angina that can be caused by high cholesterol.  

How does Zimybe work?

Zimybe tablets 10/20mg contain a fixed dose combination of two cholesterol-lowering medications that work by different mechanisms to lower high levels of cholesterol (hypercholesterolaemia) to reduce risk of heart disease.  Cholesterol is needed by the body for many functions such as building cell membranes, producing certain hormones and transporting fat soluble vitamins around the body.  However, if there is more cholesterol than the body needs, it becomes deposited in the artery walls where it can build up, blocking the arteries (known as plaque formation) and increasing risk of heart disease.  Ezetimibe in Zimybe tablets 10/20mg acts on the small intestine to inhibit intestinal absorption of dietary cholesterol, including plant sterols (phytosterols).  Ezetimibe in Zimybe tablets 10/20mg acts directly on the sterol transporter in the intestine wall that controls the uptake of cholesterol and phytosterols from the diet to inhibit the absorption of cholesterol.  This action of ezetimibe in Zimybe tablets 10/20mg decreases dietary cholesterol, which leads to a reduction of cholesterol stores in the liver and this helps lower blood cholesterol levels.  Simvastatin in Zimybe tablets 10/20mg belongs to a group of cholesterol-lowing medications called statins and works by inhibiting HMG-CoA reductase, an important enzyme which is needed for cholesterol synthesis in the liver.  This action of simvastatin in Zimybe tablets 10/20mg reduces the production of cholesterol in the liver, however, it only blocks cholesterol that is produced in the liver and has no effect on cholesterol that comes from fat in food, which is why a low-fat diet is important even when you are taking Zimybe tablets 10/20mg.  

What does Zimybe contain?

Zimybe tablets 10/20mg contain a fixed combination of the active ingredients ezetimibe 10mg and simvastatin 10mg that are used together to lower high levels of cholesterol (hypercholesterolaemia) to reduce risk of heart disease. They also contain lactose, citric acid, butylated hydroxylanisole, ascorbic acid, croscarmellose sodium, hypromellose, sodium lauryl sulphate, microcrystalline cellulose, magnesium stearate.

Effects of alcohol and grapefruit while taking Zimybe

Alcohol if drunk is large quantities may increase the risk of liver problems when taking Zimybe tablets 10/20mg.  Grapefruit juice in large quantities (more than one litre a day) can also increase risk of side effects as it increases the amount of Zimybe tablets 10/20mg that gets into your blood. 

What are the side effects of Zimybe?

The most commonly reported side effects when taking Zimybe tablets 10/20mg include constipation, diarrhoea, abdominal pain, nausea, headache and dizziness.  Less common side effects may include tenderness or weakness, tingling in the hands or feet (paraesthesia), insomnia, fever, skin rash, itchiness, hives, painful, swollen joints, anaemia, peripheral oedema.  A more serious but rare side effect is muscle pain not caused by exercise.

When should Zimybe not be used?

You should not use Zimybe tablets 10/20mg if you:

  • are allergic to ezetimibe or simvastatin or any ingredients in Zimybe
  • are pregnant or are breastfeeding
  • have liver disease
  • have severe lung disease
  • have myopathy (unexplained muscle pain) that is not due to taking other lipid lowering agents
  • are taking medicines that interact with Zimybe other cholesterol-lowering drugs such as bile acid sequestrants like cholestyramine and fibrates like clofibrate, the immunosuppressant cyclosporine, the anticoagulants warfarin, niacin (vitamin B3), macrolide antibiotics like clarithromycin, erythromycin, HIV protease inhibitor, such as ritonavir, the antifungals itraconazole and ketoconazole, calcium channel blockers like verapamil or diltiazem for high blood pressure, angina, or other heart conditions, digoxin for heart failure, ticagrelor for treating heart attack

What medications interact with Zimybe?

Several medications interact with Zimybe tablets 10/20mg and should either not be taken while you are taking Zimybe or only after discussion and instruction from your doctor:

  • Drugs that cause a serious reaction with Zimybe: niacin (vitamin B3) can increase risk of myopathy/rhabdomyolysis (muscle pain and destruction of skeletal muscle due to simvastatin, macrolide antibiotics like clarithromycin, erythromycin, HIV protease inhibitor, such as ritonavir, itraconazole and ketoconazole use to treat fungal infection, ticagrelor for treating heart attack
  • Drugs that affect Zimybe: other cholesterol lowering medication such as gemfibrozil, the immunosuppressant cyclosporin, calcium channel blockers like verapamil or diltiazem for high blood pressure, angina, or other heart conditions, the antiarrhythmic amiodarone, anticoagulants such as warfarin
  • Drugs that are affected by Zimybe: digoxin for heart failure

How should Zimybe be taken?

You should take your Zimybe tablets 10/20mg swallowed whole with a glass of water once a day, with or without food preferably in the evening, as simvastatin is most effective at night when your body is resting and your liver is producing the most cholesterol.  The dose you take may be adjusted by your doctor depending on how well you respond.  It may take up to 2 weeks before your cholesterol levels begin to fall and you should continue to take Zimybe tablets 10/20mg for as long as recommended by your doctor, which may be for the rest of your life as they will help keep your cholesterol levels from being too high but if you stop taking your Zimybe tablets 10/20mg, your cholesterol levels may rise again.

Missed dose of Zimybe

If you miss a dose of Zimybe tablets 10/20mg take it as soon as you remember, unless it is time to take the next dose, then skip the missed dose.  Do not take a double dose.

How should Zimybe be stored?

You should store your Zimybe tablets 10/20mg below 25°C in a cool dry place.
