What is Plendil used for?

Plendil tablets 5mg are used to lower blood pressure in people with hypertension (high blood pressure), who are at risk of cardiovascular disease due to high blood pressure. Plendil ER tablets 5mg can either as the first line treatment or in combination with other antihypertensive drugs if normal blood pressure has not been achieved with other medication. Felodipine tablets 5mg are also used to treat chronic stable angina, to relieve and prevent symptoms like chest pain and breathlessness; also to prevent heart attack. Plendil tablets 5mg are not suitable for acute angina, which requires a different type of medication to relieve symptoms.

How does Plendil work?

Plendil tablets 5mg contain felodipine, a calcium channel blocker that inhibits the inflow of calcium ions across specific calcium channels in the cell membrane into smooth muscle cells of the heart (myocardium) and blood vessel walls (only arteries). Calcium is needed for smooth muscle cells to contract, so by blocking calcium channels, smooth muscle cell contraction is prevented. The result is that the smooth muscle cells relax, which causes peripheral arteries to widen (vasodilation) and this reduces the workload on the heart, which is the force needed for the heart to pump blood around the body (negative inotropic effect). The result of this reduced resistance in peripheral blood vessels is to increase oxygen supply to the heart and decrease blood pressure. Widening of the main arteries of the heart also increases the oxygen supply to the heart and, as the heart workload is reduced, the demand for oxygen is also reduced, which prevents chest pain symptoms of angina caused by lack of oxygen to the heart muscle. Plendil tablets 5mg are formulated for controlled extended release, which means that they are effective over a 24 hour period.

What does Plendil contain?

Plendil tablets 5mg contain the active ingredient felodipine, a calcium channel blocker used to treat hypertension and angina. They also contain hydrogenated castor oil, propyl gallate, hypromellose, sodium aluminium silicate, microcrystalline cellulose, lactose anhydrous, sodium stearyl fumarate, hydroxypropyl cellulose. The coating on each tablet contains macrogol 6000, titanium dioxide, iron oxide yellow, iron oxide red, carnauba wax.

Felodipine blocks the inflow of calcium ions into smooth muscle cells of the heart and blood vessel walls (only arteries). Since calcium ions are needed for contraction of smooth muscle, this action causes blood vessel walls to relax and peripheral arteries to widen. The action of felodipine in Plendil tablets 5mg reduces the workload on the heart and its demand for oxygen, while at the same time increasing oxygen supply to the heart, which prevents chest pain symptoms of angina caused by lack of oxygen to the heart muscle. Plendil tablets 5mg also reduces breathlessness and improves exercise tolerance and can be used as monotherapy or with other anti-angina medications like beta-blockers.

What are the side effects of Plendil?

The most commonly reported side effects when taking Plendil tablets 5mg include: headache oedema (swelling of the ankles, feet, face or hands), flushing, palpitations, tiredness, drowsiness, dizziness, abdominal pain and nausea.

When should Plendil not be used?

You should not use Plendil tablets 5mg if you:
  • are allergic to felodipine, dihydropyridines (other calcium channel blockers) or any of the ingredients in Plendil tablets
  • are pregnant or breastfeeding
  • have unstable angina, uncompensated heart failure or acute MI (heart attack)
  • have liver disease
  • are taking medicines that interact with Plendil, including: cimetidine for ulcers, antibiotics like erythromycin, anti-fungals like itraconazole and ketoconazole, barbiturates, antiinfectives like rifampicin, antiepieptics like phenytoin and carbamazepine, flavonoids present in grapefruit juice

What medications interact with Plendil?

Several medications interact with Plendil tablets 5mg and should either not be taken while you are taking Plendil or only after discussion and instruction from your doctor:
  • Drugs that affect Plendil: cimetidine for ulcers, antibiotics like erythromycin, anti-fungals like itraconazole and ketoconazole, barbiturates, antiinfectives like rifampicin, antiepileptics like phenytoin and carbamazepine, flavonoids present in grapefruit juice
  • Drugs that are affected by Plendil:the immunosuppressant tacrolimus

How should Plendil be taken?

You should take your Plendil tablets 5mg with a glass of water, swallowed whole, once daily in the morning, without food or following a light meal not rich in fat or carbohydrate. If required your doctor may increase your dose, depending on what you are being treated for and how well you respond. You should continue to take your Plendil tablets 5mg for as long as recommended by your doctor.

Missed dose of Plendil

If you miss a dose of Plendil tablets 5mg take it as soon as you remember, unless it is time to take the next dose, then skip the missed dose. Do not take a double dose.
