What is Trelegy Ellipta 100/62.5/25 used for?

Trelegy Ellipta 100/62.5/25 is an inhaler that combines three medicines in one device to help you breathe easier and manage long-term symptoms of chronic obstructive pulmonary disease (COPD) and certain cases of asthma. It is a once-daily triple therapy for COPD and asthma.

  • Adults with COPD (including chronic bronchitis or emphysema).
  • Adults with asthma not adequately controlled by other inhaled medications.

It is not suitable for use as a rescue inhaler

How does Trelegy Ellipta 100/62.5/25 work?

Trelegy Ellipta 100/62.5/25 is a triple combination inhaler used to manage chronic obstructive pulmonary disease (COPD) and, certain cases asthma. It works by combining three active medicines that target different parts of the airways to improve breathing and prevent flare-ups.

  1. Fluticasone furoate (100 micrograms) – Inhaled corticosteroid (ICS)

Reduces inflammation and swelling in the airways and helps prevent irritation and narrowing of the airways, making it easier to breathe and reducing the risk of COPD or asthma attacks.

  1. Umeclidinium (62.5 micrograms) – Long-acting muscarinic antagonist (LAMA)

Blocks certain receptors (muscarinic receptors) in the muscles around the airways which relaxes and opens the airway muscles, allowing more air to flow in and out of the lungs. This helps relieve symptoms like wheezing, coughing, and shortness of breath.

  1. Vilanterol (25 micrograms) – Long-acting beta₂-agonist (LABA)

Stimulates beta₂-receptors in the airway muscles, causing them to relax which keeps the airways open for up to 24 hours, improving breathing and reducing chest tightness.

Together, these three medicines:

  • Open the airways and keep them open for a full day
  • Reduce inflammation and mucus buildup
  • Improve lung function and overall breathing
  • Decrease the frequency of flare-ups (exacerbations)

Trelegy Ellipta is not a rescue inhaler — it doesn’t work immediately to relieve sudden breathing problems. It’s a maintenance treatment, taken once daily, to keep symptoms under control long-term.

What does Trelegy Ellipta 100/62.5/25 contain?

Each inhalation delivers:

  • Fluticasone furoate (100 mcg) – a corticosteroid that reduces inflammation in the airways.
  • Umeclidinium (62.5 mcg) – a long-acting muscarinic antagonist (LAMA) that helps open the airways by relaxing airway muscles.
  • Vilanterol (25 mcg) – a long-acting beta₂-agonist (LABA) that further relaxes airway muscles to improve airflow.

Together, these medicines work throughout the day to control symptoms such as breathlessness, wheezing, and coughing.

What are the side effects of Trelegy Ellipta 100/62.5/25?

Common side effects may include:

  • Headache
  • Sore throat or hoarse voice

  • Oral thrush (yeast infection in the mouth)

When should Trelegy Ellipta 100/62.5/25 not be used?

1. Allergy or hypersensitivity

Do not use Trelegy Ellipta if you are allergic to:

  • Any of its active ingredients: fluticasone furoate, umeclidinium, or vilanterol
  • Or to milk proteins (since the inhaler contains lactose)

Signs of an allergic reaction may include:

  • Rash, itching, or hives
  • Swelling of the face, lips, tongue, or throat
  • Difficulty breathing

If these occur, stop using the inhaler and seek medical attention immediately.

 2. Sudden or severe breathing problems (acute symptoms)

Trelegy Ellipta should not be used for sudden asthma attacks or acute COPD flare-ups.
It is a maintenance inhaler, not a rescue inhaler.
If you have sudden shortness of breath or wheezing, you need a fast-acting (reliever) inhaler, such as salbutamol (Ventolin).

 3. Use in children or adolescents

Trelegy Ellipta is not approved for use in children or adolescents under 18 years of age.
It is only indicated for adults with:

  • Chronic obstructive pulmonary disease (COPD), or
  • Asthma (in certain formulations)

 4. Certain medical conditions – use with caution or avoid

Trelegy Ellipta should be used carefully or not at all if you have:

  • Severe heart problems (e.g., arrhythmia, coronary artery disease, or heart failure)
  • High blood pressure
  • Overactive thyroid (hyperthyroidism)
  • Seizure disorders
  • Narrow-angle glaucoma
  • Enlarged prostate or urinary retention
  • Active or untreated infections, especially tuberculosis, fungal, bacterial, or viral infections of the lungs
  • Weakened immune system or long-term steroid use

Always tell your doctor about these before starting treatment.

What medications interact with Trelegy Ellipta 100/62.5/25?

Trelegy Ellipta (fluticasone furoate, umeclidinium, and vilanterol) may interact with other medications through several mechanisms, including CYP3A4 metabolism, additive cardiovascular or anticholinergic effects, and drug–drug pharmacodynamic interactions. Below are some examples of specific drugs and classes that can interact with Trelegy Ellipta.

Ketoconazole, itraconazole, voriconazole, etc. (strong CYP3A4 inhibitors) Increased fluticasone exposure and risk of corticosteroid side effects.

Ritonavir, lopinavir, other HIV protease inhibitors Similar risk of increased corticosteroid exposure and systemic side effects. These medications inhibit CYP3A4.

Clarithromycin, telithromycin May potentiate corticosteroid exposure and related side effects. They are CYP3A4 inhibitors.

Pseudoephedrine, phenylephrine (decongestants, sympathomimetics) Additive cardiovascular stimulation such as increased heart rate or blood pressure.

Loop diuretics / thiazide diuretics

Increased risk of hypokalemia (low potassium). Monitor electrolytes if used together.

Non-selective β-blockers (e.g. propranolol) May block bronchodilator effect or trigger bronchospasm.Use with caution, especially in asthma or COPD patients

Other inhaled LABAs / long-acting β₂-agonists Risk of overdose or excessive β-agonist stimulation

How should Trelegy Ellipta 100/62.5/25 be taken? Follow your Doctor or Healthcare provider’s instructions exactly — this medicine is for maintenance, not for sudden breathing problems.

  • Use one inhalation once daily, at the same time each day.
  • Do not use more than once in 24 hours.
  • Rinse your mouth with water after use to help prevent oral thrush (a common side effect of inhaled steroids).

Missed dose of Trelegy Ellipta 100/62.5/25 If you miss a dose of Trelegy Ellipta

  • Take it as soon as you remember.
  • If it’s almost time for your next dose, skip the missed one and just take your next dose at the regular time.
  • Do not take more than one dose in a day to make up for the missed dose.

How should Trelegy Ellipta 100/62.5/25 be stored?

Store in a cool, dry place below 30°C and keep out of reach of children.
