Ismo 40 retard general information

What is Ismo 40 retard used for?

Ismo 40 retard tablets 40mg are used for long-term treatment of various chronic cardiovascular conditions.  These include coronary heart disease (CHD), caused by clogged arteries in the heart (atherosclerosis) due to build-up of fatty deposits (atherosclerotic plaque) in the walls of the arteries.  CHD can cause angina, heart attack (myocardial infarction) and can result in coronary death.  Ismo 40 retard tablets are also used as prophylaxis to prevent attacks of acute angina pectoris, characterised by symptoms including chest pain and breathlessness on exertion, but not to treat an angina attack.  Ismo 40 retard tablets are also used to treat chronic congestive heart failure in which the heart cannot pump efficiently usually due to blocked or narrowed blood vessels, in addition to treatment with other heart medications.  Ismo 40 retard tablets are also used to treat pulmonary hypertension, which is increased blood pressure in the blood vessels of the lungs, caused by narrowing of the small vessels of the lungs.  This increases pressure on the right ventricle of the heart, which becomes weaker and could eventually fail.  Symptoms include shortness of breath, dizziness, feinting (syncope) and peripheral oedema (swelling in the legs, ankles and feet).  Ismo 40 retard tablets 40mg work by the same mechanism to treat these cardiovascular conditions and prevent worsening of symptoms of chronic heart conditions. 

How does Ismo 40 retard work?

Ismo 40 retard tablets 40mg contain isosorbide-5-mononitrate, a potent vasodilator used to relax and widen peripheral arteries and veins that is used to treat chronic cardiovascular disease and prevent the onset of acute episodes.  Isosorbide-5-mononitrate in Ismo 40 retard tablets 40mg acts directly on the smooth muscles of blood vessels and is thought to work by being converted to nitric oxide, which stimulates the production of guanosine monophosphate (cGMP), and this mediates relaxation of smooth muscle and causes them to widen or dilate.  Dilatation of peripheral veins in particular, causes increased capacity or pooling of blood and thereby reduces pressure of the blood returning to the heart and lungs (known as preload).  Relaxation of peripheral arteries similarly reduces pressure in the arteries taking blood from the heart and lungs (known as afterload).  Isosorbide-5-mononitrate in Ismo 40 retard reduces the resistance to blood flow in the peripheral veins and arteries which means that the heart does not need to work so hard to pump blood, reducing its workload and also the need for oxygen by the heart muscle.  This action of Ismo 40 retard helps prevent acute angina caused by insufficient blood to the heart caused by restricted blood flow due to narrowing of the arteries of the heart (coronary arteries), because of a blockage due to deposition of cholesterol in the vessel walls (atherosclerosis),  and also relieves symptoms of congestive heart failure.  The same mechanism of action widens blood vessels in the lungs to relieve symptoms of pulmonary hypertension.  Ismo 40 retard tablets 40mg are formulated for prolonged release so that the active medication is released over several hours for prolonged effectiveness.

What does Ismo 40 retard contain?

Ismo 40 retard tablets contain the active ingredient isosorbide-5-mononitrate, a vasodilator used for long-term treatment of chronic cardiovascular disease.  They also contain glucose syrup, lactose, macrogol 35,000, magnesium stearate (Ph.Eur.), montanglycol wax, povidone K25, poly[butyl methacrylate-co-(2- dimethylaminoethyl)methacrylate-co-methyl methacrylate] (1:2:1), sucrose, highly dispersed silica, talc, kaolin and titanium dioxide (E171).

Treating angina with Ismo 40 retard

Ismo 40 retard tablets 40mg contain isosorbide-5-mononitrate, a potent vasodilator used to relax and widen peripheral arteries and veins, and is used as prophylaxis for chronic angina to prevent angina attacks, in addition to treatment with other heart medications, but not to treat an angina attack.  Angina is usually an indication of coronary heart disease (CHD) and is caused by insufficient blood to the heart due to a reduction of blood flow.  This is usually due to narrowing of the arteries of the heart (coronary arteries) because of a blockage caused by deposition of cholesterol in the vessel walls (atherosclerosis).  Isosorbide-5-mononitrate in Ismo 40 retard acts directly on the vascular smooth muscle in the blood vessel walls, causing them to dilate and widen.  This action reduces the resistance to blood flow in the peripheral veins and arteries which means that the heart does not need to work so hard to pump blood, reducing its workload and also the need for oxygen.  Isosorbide-5-mononitrate in Ismo 40 retard tablets 40mg also causes widening of the coronary arteries, which helps get more oxygen to the heart muscle and relieves symptoms of acute angina attack including pain in the chest, referred pain to the arms or neck and breathlessness on exertion.   

Treating pulmonary hypertension with Ismo 40 retard

Ismo 40 retard tablets 40mg contain isosorbide-5-mononitrate, a potent vasodilator used to relax and widen peripheral arteries and veins, and is used to treat pulmonary hypertension to relieve symptoms including shortness of breath, dizziness, feinting (syncope) and peripheral oedema (swelling in the legs, ankles and feet).  Pulmonary hypertension is increased blood pressure in the blood vessels of the lungs, caused by blockage and narrowing or damage to of the small vessels of the lungs.  This increases resistance in the blood vessels in the lungs and raises blood pressure in the lungs; also pressure on the right ventricle of the heart, which becomes weaker and could eventually fail.  Isosorbide-5-mononitrate in Ismo 40 retard acts directly on the vascular smooth muscle in the blood vessel walls, causing them to dilate and widen.  This action reduces the resistance to blood flow in the small arteries and capillaries in the lungs, which helps relieve symptoms of pulmonary hypertension and also reduces the workload on the heart and its need for oxygen, which means that the heart does not need to work so hard to pump blood.

What are the side effects of Ismo 40 retard?

The most commonly reported side effects when taking Ismo 40 retard include, headache, light-headedness, dizziness, tiredness, nausea and vomiting, low blood pressure (hypotension), rapid heartbeat (tachycardia), flushing.

When should Ismo 40 retard not be used?

You should not use Ismo 40 retard tablets 40mg if you:

  • are allergic to isosorbide-5-mononitrate or any ingredients in Ismo 40 retard
  • are pregnant or are breastfeeding
  • have acute circulatory failure (shock, circulatory collapse), cardiogenic shock, hypertrophic obstructive cardiomyopathy, (disease of the heart muscle), or constrictive pericarditis (inflammation of the pericardium) and pericardial tamponade (build up or fluid around the heart)
  • have low blood pressure (hypotension)
  • have acute angina or acute heart attack
  • have hereditary problems of glucose-galactose malabsorption, sucrase-isomaltase deficiency galactose intolerance, or fructose or lactase deficiency
  • are taking medicines that interact with Ismo 40 retard, including phosphodiesterase-5 inhibitors - including like sildenafil for erectile dysfunction, ACE inhibitors, calcium channel blockers and beta blockers like propranolol for high blood pressure also diuretics, neuroleptics or tricyclic antidepressants and alcohol, dihydroergotamine for migraine

What medications interact with Ismo 40 retard?

Several medications interact with Ismo 40 retard and should either not be taken while you are taking Ismo 40 retard or only after discussion and instruction from your doctor:

  • Drugs that cause a serious reaction with Ismo 40 retard: phosphodiesterase-5 inhibitors - including like sildenafil for erectile dysfunction, ACE inhibitors, calcium channel blockers and beta blockers like propranolol for high blood pressure also diuretics, neuroleptics or tricyclic antidepressants, alcohol, dihydroergotamine for migraine can potentiate the antihypertensive effect of ISMO 40 retard.
  • Drugs that are affected by Ismo 40 retard: dihydroergotamine for migraine

How should Ismo 40 retard be taken?

You should take your Ismo 40 retard tablets 40mg swallowed whole with a glass of water before or after food, once daily in the morning and at the same time each day.  The dose you take and how often depends on what you are being treated for and your doctor’s recommendation and may be increased to twice daily.  You should continue to take your Ismo 40 retard tablets 40mg for as long as recommended by your doctor, which may be long term, as they will help treat your symptoms but will not cure your condition.

Missed dose of Ismo 40 retard

If you miss a dose of Ismo 40 retard tablets 40mg take it as soon as you remember, unless it is time to take the next dose, then skip the missed dose.  Do not take a double dose.
