Ponstan contains mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID) that is used to treat dysmenorrhoea, menorrhagia and for symptomatic pain relief in various inflammatory conditions.

What is Ponstan used for?

Ponstan capsules 250mg are used to treat primary dysmenorrhoea, which is period pain or menstrual cramps, with symptoms including painful cramping in the lower abdomen, generalised pain in the lower abdomen and lower back, and nausea.  These symptoms usually last for a few days around the time of menstruation.  Ponstan capsules 250mg are also used to treat dysfunctional uterine bleeding, which is bleeding from the vagina that is not related to a disease or a pregnancy but is usually due to a disruption in the normal hormone cycle.  Ponstan capsules 250mg can also be used for pain or menorrhagia (heavy periods) caused by using an intrauterine contraceptive device (IUCD).  Ponstan capsules 250mg can also be an effective analgesic for symptomatic pain relief due to other forms of pain, including arthritic pain of rheumatoid arthritis and osteoarthritis, soft tissue pain such as sprains and strains due to injury; also dental pain.  Ponstan capsules 250mg work by the same mechanism to reduce pain and inflammation in these various conditions.

How does Ponstan work?

Ponstan capsules 250mg contain mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID) that has anti-inflammatory, analgesic (pain relieving) and anti-pyretic properties (reduces fever).  Mefenamic acid in Ponstan capsules 250mg block the synthesis of specific chemicals called prostaglandins (PG), particularly PGE2 and PGF2α that mediate pain and inflammation.  These prostaglandins are thought to stimulate uterine cramping that is the main cause of primary dysmenorrhoea.  Mefenamic acid in Ponstan capsules 250mg works by inhibiting the enzyme cyclooxygenase (COX) that is involved in the synthesis of prostaglandins and acts on both cyclooxygenase 1 (COX-1) and cyclooxygenase 2 (COX-2).  It is COX-2 that is induced during the inflammation process and is responsible for the synthesis of inflammatory prostaglandins.  Inhibition of COX-2 helps relieve the symptoms of inflammation, including pain, swelling, and joint stiffness.  Inhibition of COX-1 is responsible for the gastrointestinal side effects that are due to inhibition of protective prostaglandins in the gut and disruption of regulation of acid content.  Mefenamic acid in Ponstan capsules 250mg is also thought to act at the site of prostaglandin receptors to inhibit the response to prostaglandins.  This dual action of Mefenamic acid in Ponstan capsules 250mg provides better pain relief than other NSAIDs that only block the synthesis of prostaglandins.

What are the side effects of Ponstan?

The most commonly reported side effects when taking Ponstan capsules 250mg are gastrointestinal and include: upset stomach or stomach pain, diarrhoea, nausea, indigestion, bloating wind, heartburn, indigestion, constipation.  Other less common side effects include: dizziness, drowsiness, headache, light-headedness, blurred vision, oedema and fluid retention, skin eruptions including rash, itching, sweating.

When should Ponstan not be used?

You should not use Ponstan capsules 250mg if you:

  • are allergic to mefenamic acid or any ingredients in Ponstan
  • are pregnant or are breastfeeding, without discussion with your doctor
  • have severe kidney or liver disease
  • have aspirin-induced asthma
  • have uncontrolled hypertension, severe heart failure or cardiovascular disease, without discussion with your doctor
  • have a tendency to bleed easily
  • have a history of gastrointestinal bleeding or perforation related to other NSAID treatment
  • have active, or history of recurrent peptic ulcer/haemorrhage 
  • are taking medicines that interact with Ponstan, including aspirin or other NSAIDs, anticoagulants like warfarin, beta blockers and ACE inhibitors for high blood pressure, diuretics like frusemide, corticosteroids, immunosuppressive like tacrolimus, insulin or oral hypoglycaemic agents for diabetes, lithium for mood disorders, anti-cancer drug methotrexate

What medications interact with Ponstan?

Several medications interact with Ponstan capsules 250mg and should either not be taken while you are taking Ponstan or only after discussion and instruction from your doctor:

  • Drugs that cause a serious reaction with Ponstan: aspirin or other NSAIDs, anticoagulants like warfarin, corticosteroids, immunosuppressive like tacrolimus, lithium for mood disorders
  • Drugs that are affected by Ponstan: aspirin, beta blockers and ACE inhibitors for high blood pressure, diuretics like frusemide, insulin or oral hypoglycaemic agents for diabetes, lithium for mood disorders, anti-cancer drug methotrexate

How should Ponstan be taken?

You should take your Ponstan capsules 250mg swallowed whole with a meal three times daily.  For dysmenorrhoea and menorrhagia you should start taking your Ponstan capsules 250mg at the onset of menstruation or menstrual pain and not for longer than 7 days or as recommended by your doctor.  For other conditions you should take your Ponstan capsules 250mg swallowed whole with a meal three times daily for as long as recommended by your doctor.

How long should you take Ponstan?

You should take your Ponstan capsules 250mg depending on what you are taking it for and for as long as recommended by your doctor.

Missed dose of Ponstan

If you miss a dose of Ponstan take capsules 250mg it as soon as you remember, unless it is time to take the next dose, then skip the missed dose.  Do not take a double dose.
